The Care Coordinator may be called upon to cover shifts him/herself when COHH is short on replacement staff. Care Coordinator’s can then adjust their schedules in the next couple days/week to accommodate the extra hours of work. The Care Coordinator is to be available to COHH clients, office, and administration 24/7. Care Coordinator’s must have a 4-year degree in Social Services or have 5 years of experience in a supervisory role in the healthcare field.

If kept on as a permanent Care Coordinator , annual job performance evaluations will be conducted thereafter. The Care Coordinator is a supervisory role for Comfort of Home Healthcare and is responsible for the daily operation in the homes that they are assigned.
